Agreed. But only if they are capable of being fixed, if they can't or they are having too much trouble trying to do so, then I would suggest getting rid of them or starting the code all over again.
However I do think there are some achievements that are too overly ridiculous (but not impossible) to get for any sane human being, and some times I think if atleast 1/4 of the current medal games we have really deserve their medals (or atleast specific achievements).

The only thing I disagree about your petition is the fight over the Literate/Iliterate medals in SOD. Personally, I think it's a fun idea to give people a choice like that. I hope it's implimented in many games to come. I do understand the whole thing about faulty internet connections and server problems, but, as in the case with matt-likes-swords, it can be fixed, and I believe that resend buttons like his should be mandatory.
Having said that, I do agree on every other point with you. Faulty medals should be removed and future authors should have to test and debug thoroughly before publishing the game.
